Bilateral Pneumonia - R/O Heart Disease

A 40 year old gentleman was being treated for bilateral pneumonia. As he had ongoing symptoms, he was sent for an echocardiogram which revealed Severe Calcific Mitral Stenosis. Patients being treated for “Bilateral Pneumonia “ often have underlying heart disease - ACS / Valvular Heart Disease / Myocarditis etc. An echocardiogram is mandatory prior to discharge of such patients - else the diagnosis would be missed.

Unilateral Edema -Think of DVT

A 60 year old gentleman presented with pain and swelling of the left leg. He was prescribed NSAIDs for arthralgia but came as the pain was persistent. A Doppler was suggestive of DVT and was started on Dabigatran after thrombophilia screen.

Deceiving Symptoms but catching the truth

72 year old man presented in emergency with complaints of oppressive central chest pain and aches in the back between the scapula 1 day duration. He is a long-standing diabetes and hypertensive patient on treatment. We have done an initial work up for cardiac enzymes ECG echo .Apart from mild leucocytosis others found to be normal.To rule out possibility of dissecting aneurysm CT chest done ,was normal but in the lower cuts of CT they saw a huge calculus in gall bladder with periicholecystic collection.Shifted to GI surgical side and underwent lap cholecystectomy.Discharged next day.

Be(A)ware of Neck Pain

A 55 year old lady presented with pain around her neck for 2 weeks. She was seen by the neurologist and started on treatment for spondylosis. She is a known diabetic (HbA1c 12.5) and was recently started on drugs for hypertension & dyslipidemia. As her symptoms persisted she went to another doctor who did a TMT for her which was strongly positive. Her Angiogram showed critical Left Main disease and was advised early CABG vs Angioplasty. Her Angiogram and a normal angiogram for comparison is given

Heart failure precipitated by unaccustomed exertion in an elderly male

An 82-year-old man presented it to gastroenterology OP clinic with bloating and hiccups following a physically strenuous trip to a place of worship the day prior. He had no coronary risk factors. Sinus tachycardia of 100 with ectopic beats, and a systolic murmur were found. He was suspected to have ischaemic heart disease and referred to cardiology immediately. He was diagnosed with acute coronary syndrome, NSTEMI with acute left heart failure. Troponin-I was elevated at 200.4. He was treated with diuretics, heparin and antiplatelet treatment, on which he improved rapidly.
