Parasitic disease mimicking bladder tumor

35-year-old African male patient was seen in my clinic for mild lower urinary tract symptoms. On imaging with Ultrasound and CT scan, a one cm bladder mass was found at the dome of the bladder. Cystoscopy revealed a nodular mass(Fig 1) with features of bladder tumor and was resected completely. Histopathology revealed Schistosomiasis of the bladder. The attached picture(Fig 2) shows the eosinophilic infiltrate and ovoid Schistosoma eggs. This is a rare presentation of the parasitic disease. The most common features are inflammatory in nature.

An educated and well read 64 year old female type 1diabetic for the past 43 years controlled on basal bolus regime routinely operates her glucometer to self check blood glucose even during severe hypoglycemia. At times the reading is in 30's and a few times even below that. On seeing the low numbers she casually fixes a sugary drink and goes about her routine as if nothing has happened. Her HbA1c levels are usually between 7&8

73 year old male, admitted under the physician with high fever, chills uncontrolled Diabetes Mellitus, CAD. A comprehensive battery of investigations for PUO, was done but no specific cause of fever could be identified .Patient was put on Meropenam 1 gm Q8 hrly. Plain and Contrast CT Abdomen and Pelvis: normal. Urology reference for mild dysuria and frequency was sought. P. R : grade 2 non tender nodular prostatomegaly of varying consistency.
